Omnizole-D interacts with several common medicines and calls for extra caution in heart, liver and kidney conditions. Here is what your doctor weighs before prescribing it.
Who needs extra caution with Omnizole-D
Certain groups need a closer look before this combination is prescribed.
- People with heart rhythm problems. A history of an irregular heartbeat, a long QT interval on an ECG, heart failure, or a family history of sudden cardiac death makes the domperidone component more of a concern.
- People with liver disease. Domperidone is broken down in the liver, and moderate to severe liver impairment is a reason to avoid it. Esomeprazole doses are often reduced in significant liver disease.
- People with kidney disease. The kidneys clear part of what circulates, and rare kidney inflammation has been linked to proton pump inhibitors.
- Older adults. Age brings more medicines, more heart risk and more sensitivity to both molecules, so the shortest useful course is preferred.
- People with low potassium or magnesium. These electrolyte problems raise the heart rhythm risk and are worth correcting first.
- People with bleeding, blockage or perforation in the gut. A prokinetic is not appropriate in those situations.
Omnizole-D in pregnancy and breastfeeding
Acidity in pregnancy is extremely common, and it is also a situation where self-medication is a poor idea. Domperidone in particular is not routinely recommended during pregnancy, and small amounts pass into breast milk.
If you are pregnant, planning a pregnancy or breastfeeding, say so before the prescription is written, not after the strip is opened. Doctors have several approaches for reflux in pregnancy, including dietary measures, posture advice and medicines with a longer record of use in pregnant women.
The same applies if you become pregnant during a course. Contact your doctor for a review rather than deciding on your own whether to continue.
Medicines that interact with esomeprazole and domperidone
Show your doctor and pharmacist a full list, including anything bought over the counter. The interactions clinicians watch for include:
- Clopidogrel. Esomeprazole can reduce how well this blood thinner is activated, which matters a great deal after a stent or a heart attack.
- Ketoconazole, itraconazole, erythromycin, clarithromycin. These can raise domperidone levels and also carry their own effect on heart rhythm.
- Ritonavir and some other HIV medicines. Acid suppression changes how well several of them are absorbed, in either direction.
- Antifungals and other drugs that need stomach acid to dissolve. Less acid can mean less absorption.
- Methotrexate. Levels may rise when a proton pump inhibitor is used, particularly at higher methotrexate doses.
- Digoxin, diuretics, some antidepressants and some antipsychotics. These affect either electrolytes or heart rhythm, and both matter here.
- Iron salts, some calcium supplements and vitamin B12. Absorption may fall over longer courses.
- Anticholinergic medicines. They oppose the prokinetic action, so the two work against each other.
Food, alcohol and daily habits during treatment
There is no dramatic food restriction with this combination, but a few habits pull against the treatment.
Alcohol relaxes the valve at the lower end of the food pipe and irritates the stomach lining, so reflux tends to worsen. It also adds to the load on the liver, which handles domperidone. Limiting or avoiding alcohol during a course is sensible, and your doctor can advise on your own situation.
Smoking has a similar effect on that valve and slows healing. Very late dinners, large oily meals, deep-fried festival food, strong coffee, aerated drinks and lying down soon after eating all make symptoms more likely. Grapefruit juice can interfere with liver enzymes that process several medicines and is best avoided while on this capsule.
Tests your doctor may order alongside Omnizole-D
Depending on your history and how long treatment runs, a doctor may ask for:
- an ECG, especially in older adults, in heart disease, or when other rhythm-affecting medicines are in use
- serum magnesium and potassium, particularly with longer courses or with diuretics
- vitamin B12 for extended proton pump inhibitor use
- liver and kidney function tests
- a test for Helicobacter pylori, or an endoscopy, where the diagnosis needs confirming
Acid suppression can also mask the appearance of some conditions at endoscopy, so tell any specialist that you are on this capsule before a scheduled scope.
