Atherosclerotic disease has two related problems. Cholesterol-rich plaque builds up in artery walls, and platelets can form a clot on a plaque that cracks. Statins address the first problem and antiplatelet drugs address the second.

Aspirin and clopidogrel are used together because they act on separate activation pathways, COX-1 and P2Y12. Blocking one pathway leaves the other open, so the pairing gives broader platelet suppression than either alone.
