In practice the 10 mg strength is chosen for a gentle start, for older patients, for those with kidney disease or previous statin intolerance, and for maintenance once target is reached. Atomni-20 is the common step-up when 10 mg falls short and a frequent starting strength for a clearly raised LDL. Atomni-40 is high-intensity therapy for people who have had a heart attack, stroke or stent, or who carry very high risk, where guidelines call for the largest LDL reduction from the outset.

Rosuvastatin is more potent milligram for milligram, so Rosomni-10 lowers LDL more than Atomni-10 despite the same number on the strip. It is water-soluble and cleared with little involvement of the CYP3A4 enzyme, which makes it a preferred option for patients taking clarithromycin, azole antifungals, HIV protease inhibitors or cyclosporin.
