Nicomni-5 Precautions and Drug Interactions

Nicorandil relaxes blood vessels, so anything that already lowers blood pressure or blood volume increases the chance of dizziness and fainting. Tell your doctor if any of the following apply.

None of these automatically rules out treatment. They simply change how carefully a doctor starts and monitors it.

There are conditions in which nicorandil is usually avoided altogether. These include cardiogenic shock, severe low blood pressure, low blood volume, and acute fluid overload in the lungs. Heart failure where the filling pressure inside the heart is already low is another situation where doctors are cautious, because further vessel relaxation can reduce output rather than help.

A known allergy to nicorandil or to any of the excipients in the tablet is also a reason not to use it.

This is exactly why Nicomni-5 sits under Schedule H. A chemist cannot judge these situations across a counter, and a prescription written for one person is not transferable to a family member with similar symptoms. Sharing heart medicines within a household is a genuine risk, not a technicality.

Some combinations matter a great deal.

Always confirm any new medicine with the doctor treating your heart, including one prescribed by a different specialist.

Alcohol widens blood vessels on its own. Taken along with nicorandil, it can increase flushing, headache, dizziness and the risk of a fall. Ask your doctor directly about alcohol rather than assuming a small amount is fine, since the answer depends on your heart condition and the rest of your prescription.

Food does not usually need to be restricted for this molecule specifically. Your doctor may still advise a diet that supports heart health, including less salt, less deep fried food and more fruit and vegetables, because those measures serve the underlying coronary disease.

During long religious fasts, festivals or a heat wave, dehydration is the practical concern. Discuss fasting plans with your doctor in advance so that the timing of tablets and fluid intake can be planned together.

Hot baths, saunas and standing for long periods can worsen the light headed feeling. Rise slowly from bed and sit for a moment before standing.

Nicorandil is generally not recommended during pregnancy unless a doctor judges that the expected benefit outweighs the possible risk, because experience in pregnancy is limited. If you are pregnant, think you may be pregnant or are planning a pregnancy, tell your doctor before continuing.

For breastfeeding, information is limited, so nicorandil is usually avoided while nursing unless a specialist advises otherwise.

In older adults, doctors often begin at a modest strength and review blood pressure carefully. In kidney or liver impairment, the whole medicine list is usually reviewed together.

Nicorandil is not established for use in children, and the pack instruction to keep the medicine out of reach of children should be followed strictly.

There is no single fixed test schedule for everyone. Depending on your history, a doctor may check blood pressure and pulse at each visit, including readings taken sitting and standing, and may order an ECG, blood tests covering kidney and liver function, a complete blood count, or cardiac imaging.

They may also ask practical questions that work as a test of their own: how far you can walk before symptoms start, how many times a week you needed a rescue medicine, and whether any ulcers or stomach symptoms have appeared.

Keep your reports in one folder and carry it to appointments. If you see a dentist, a surgeon or any new doctor, mention that you take nicorandil, because mouth ulcers and gut symptoms are easy to misread when the medicine list is unknown.
