The difference is the telmisartan. Cilomni-T 10/40 adds an angiotensin receptor blocker on top of the same calcium channel blocker.
What each tablet does
Cilomni-10 acts on the artery wall. Cilnidipine relaxes vascular smooth muscle and calms sympathetic nerve endings, so pressure falls mainly through vessel widening.
Cilomni-T 10/40 does all of that and also blocks the angiotensin II receptor. That second action reduces the vessel tightening and salt retaining signal coming from the renin angiotensin system. In practice, a doctor moving a patient from the single molecule to the combination is usually adding a mechanism rather than just raising a dose, and the follow up tests change accordingly.
What a doctor considers when choosing
- How far the current readings sit from the target
- Whether one molecule has already been tried, and for how long
- Kidney function, protein in the urine and blood potassium
- Diabetes, heart disease or a previous stroke
- Pregnancy, or plans for pregnancy, which rule out telmisartan
- Side effects on the present tablet, such as ankle swelling or dizziness
- How many tablets the patient can realistically take every single day
The prescription that results is a judgement about one person, not a ranking of two products.
Why fixed dose combinations exist
Two molecules at moderate doses often control pressure more evenly than one molecule pushed to a high dose, and effects that depend on dose may be less troublesome as a result. Putting both into a single film-coated tablet also removes an item from the daily routine, which matters for a treatment that runs for years.
The limitation is that the doses are locked together. If a doctor wants to change only the telmisartan part, the combination has to be reconsidered. That is why some patients stay on separate tablets even when a combination exists.
Why patients must not switch on their own
Swapping between Cilomni-10 and Cilomni-T 10/40 without advice carries risk in both directions. Adding telmisartan yourself can drop pressure too far, raise potassium or harm a pregnancy. Dropping it yourself can let pressure climb quietly for months while you feel perfectly fine.
Both are Schedule H medicines, so a chemist should dispense either one only against a prescription that names it. If a strip handed to you does not match your prescription, ask the chemist before taking a tablet, and confirm with your doctor.
