Most people tolerate Cilomni-T 10/40 well, but side effects can happen. Here are the common, less common and serious effects of cilnidipine and telmisartan, and when to seek urgent help.

Common side effects of Cilomni-T 10/40

Most people take this combination without much trouble, but side effects can occur with any medicine. The ones reported most often with cilnidipine and telmisartan include:

  • Dizziness or light headedness, especially on standing up quickly
  • Headache in the first days of treatment
  • Flushing or a warm feeling in the face
  • Palpitations, a sense of the heart beating harder
  • Tiredness
  • Mild swelling of the ankles or feet

These usually appear early, while the body adjusts to lower pressure, and often become milder with time. Report anything that troubles you rather than stopping the tablet on your own.

Less common effects to mention at your review

  • Back pain or muscle aches, reported with telmisartan
  • Sinus congestion or a sore throat
  • Loose motions or stomach discomfort
  • Skin rash or itching
  • A rise in blood potassium, which shows on a test rather than being felt
  • A rise in creatinine, meaning the kidney test has shifted

Because the last two are silent, doctors often repeat blood tests after starting an angiotensin receptor blocker and after any dose change. That is routine care, not a sign that something has gone wrong.

Serious side effects that need urgent care

Go to a hospital or call your doctor straight away if you notice:

  • Swelling of the face, lips, tongue or throat, or difficulty breathing, which can indicate angioedema
  • Fainting, or dizziness severe enough that you cannot stand
  • Chest pain, or a fast and irregular heartbeat that does not settle
  • Very little urine passed over a whole day
  • Muscle weakness with an irregular pulse, which can accompany high potassium
  • Yellowing of the eyes or the skin

These are uncommon, but they should never be waited out at home.

Which effects usually settle?

Headache, flushing and mild dizziness at the start of treatment often ease within the first week or two as the body adapts. Standing up slowly from bed or from a chair, keeping well hydrated through a Karnataka summer, and going easy on alcohol in the early weeks all help.

Ankle swelling is a known effect of the dihydropyridine class. Cilnidipine also blocks N-type channels, and this dual action is one reason it is often described as being gentler on ankle swelling than the older members of the class. If swelling still appears, tell your doctor rather than adding a water tablet yourself.

How doctors manage side effects

A doctor may reduce a dose, shift the time of day the tablet is taken, treat the effect directly, or move to a different molecule. Sometimes another medicine you are on, such as a regular painkiller, turns out to be the real reason for a change in kidney tests or in readings.

Keep a short note of what you felt, when it began and how long it lasted. That note is far more useful at the consultation than a general complaint of not feeling well.
